Our Mission
Foundation for Health Equity™ supports communities in NW Philadelphia by building on the assets of organizations that work to address systemic health inequities in order to build healthy, vibrant neighborhoods.

We envision a community where everyone has a fair and just opportunity to achieve optimal health.
Our Guiding Principles
Green Tree Community Health Foundation was established in 2005 from the proceeds of the sale of Chestnut Hill Hospital to carry out charitable activities in the hospital’s primary service area, spanning Northwest Philadelphia and Eastern Montgomery County. In the last 15 years, the foundation has made 450 grants totaling $8.4 million to a wide array of community organizations, touching the lives of more than 495,000 neighbors.
Responding to changes in our community and city, the foundation developed a new comprehensive strategic plan in 2020 — a process both accelerated and informed by the Covid-19 pandemic and its disproportionate impact on historically marginalized communities.
In 2021, Green Tree became Foundation for Health Equity™, renewed its commitment to addressing issues of health equity, and concentrated its grantmaking in those neighborhoods of Northwest Philadelphia facing the greatest barriers to optimal health.
